The Role:

We are looking to expand our medication management team with clinical telepharmacists who share our passion for getting patient medication regimens right to drive better clinical and financial outcomes. Our vision is a world where people who need medications can access and use them safely and effectively so that they can live long healthy lives in the real world rather than in the hospital. 

You will be helping to carry out comprehensive medication reviews and chronic disease state management directly with patients, helping to keep them safe by optimizing their medication regimens.

This is a full time, remote, work from home consultant position.

Responsibilities:

  • Perform comprehensive medication management and chronic disease state management in patients with diabetes via virtual visits.
  • Optimize diabetes medication therapy in patients who are uncontrolled or have not achieved clinical goals of therapy
  • Manage and optimize other chronic disease states as needed to improve clinical outcomes.
  • Identify how patients are taking all of their medications, including OTCs, herbals, DME and provide solutions for adherence/administration, inappropriate therapy, etc. 
  • Evaluate each medication for appropriateness, effectiveness, interactions, and safety
  • Identify and manage drug therapy interventions 
  • Develop a care plan for the patient, provider, and/or payer
  • Consult with patients’ physicians to ensure clinical recommendations are considered
  • Follow up with providers on the team’s recommendations 
  • Conduct follow-up evaluations with patients and participate in other activities as assigned by the Executive Director of Clinical Services
  • Triage real-time provider and patient questions directed for the pharmacist
  • Work closely with the Executive Director of Clinical Services, Clinical Pharmacists, and Clinical Support Staff to optimize the delivery of care to patients

Requirements:

  • Graduated from an ACPE accredited school of pharmacy in the United States with a Doctor of Pharmacy degree
  • Current, unrestricted pharmacist license in Hawaii
  • Valid NPI number
  • Possess and can show proof of malpractice insurance outside of your employer
  • 3+ years of diabetes and other chronic disease state medication management in ambulatory care experience where you were managing patients 
  • Completed an ASHP accredited PGY1 Residency Program
  • Certified Diabetes Care and Education Specialist (CDCES) certification preferred
  • Actively seeing patients in your current role or within the last year
  • Comfortable creating patient medication action plans 
  • Familiar with federal and state pharmacy laws
  • Enjoy problem solving and always keep in mind what is best for the patient
  • Keep a positive attitude and genuinely enjoy talking to patients
  • Able to be focused and productive while working from home
  • Able to start working within 1 month of job offer
