Job Description
Hours of operation are 8:30am to 5:00pm.
Hybrid work model: 5-day work schedule; (3) days minimum in office, with the flexibility of working remotely on the remaining days, and/or reporting to the office for business continuity as needed.
Job purpose
Maintains regular, ongoing contact with participants, caregivers and physicians. Revisits and monitors member’s progress. Completes and submits timely paperwork and reports. Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community. Knowledge of community agencies and resources. Working knowledge of multi-systems outreach programs related to health care delivery, clinical education and health-related services. Knowledge of services and barriers to care professionals and care coordinators over the telephone. Skill in use of personal computers and related software applications, including e-mail. Skill in organizing resources and establishing priorities. Creative and analytical thinking.

Qualifications
Education:
· High school diploma or GED
· Community Health Worker certification
Experience
· 3 years of experience related to the duties and responsibilities specified.
· Knowledge of local resources for the elderly and disabled.
· Knowledge of Medicare/Medicaid regulations.
· Current Michigan driver’s license with reliable transportation to travel throughout the community.
· Computer savvy: Excel, Word, Outlook.
· Excellent communication skills, written and verbal.
· Bilingual strongly preferred.
